Community Intervention
Community Intervention services aim to reduce barriers to community integration and independent living for individuals facing mental health challenges. These services are designed to:
- Minimize the risk of hospitalization or placement in more restrictive environments.
- Facilitate collaboration with agencies, institutions, employers, landlords, and families to help individuals function more independently in their community.
- Support members in overcoming obstacles and improve their relationships with key support systems, including family members and treatment providers.
Community interventions are tailored to the individual and can be provided in person or by telephone, depending on the strategy being implemented. This service is typically provided on an individual basis to meet the specific needs of the member.
